ลักษณะผื่น

เป็น crusted or excoriated, hyperkeratotic, light to bright red papules, nodules or plaques with hyperpigmented margin

มีหลายขนาด อาจเล็กมิลลิเมตรหรือใหญ่ได้ถึง 3 เซนติเมตรมัก

ข้อสังเกต คือ มักอยู่บริเวณที่มือเอื้อมไปเกาถึง หากอยู่ที่หลัง จะเห็นบริเวณผิวหนังปกติที่เกาไม่ถึงเป็นรูปคล้ายผีเสื้อ เรียกว่า Butterfly sign 🦋

Pathogenesis

เชื่อว่าเกี่ยวข้องกับกระบวนการ neuronal sensitization, inflammation, and pruritus

Grading

mild (<20 lesions)

moderate (20-100 lesions)

severe (>100 lesions)

อย่าลืมหาสาเหตุของโรคที่ทำให้เกิดอาการคัน

ทั้งโรคผิวหนัง และ โรค systemic disease โดยเฉพาะ chronic renal disease, diabetes, neuropathic disease, chronic obstructive pulmonary disease, diabetes, congestive heart failure, high blood pressure,

อย่าลืมหาโรคร่วมอื่นที่อาจพบด้วย เช่น

atopic dermatitis, plaque psoriasis, lichen planus, or dermatitis herpetiformis

บางรายมีภาวะซึมเศร้าร่วมได้

การรักษาปรับยาตาม algorithm step ตามรูป ได้แก่

ยาทา : topical corticosteroid,calcineurin inhibitor

ยากิน : immunosuppressant, gabapentinoid, thalidomide

ยาต้านฮิสตามีนมักไม่ค่อยได้ผล

หากมีโรคประจำตัวหลายอย่าง กังวลเรื่องผลข้างเคียงของยากิน อาจใช้การฉายแสง

Emerging treatments ได้แก่

• Targeting IL-31 and its receptor IL-31RA : Nemolizumab

• Anti-IL-4RA monoclonal

antibody : Dupilumab

• Opioid antagonists and cannabinoid

• Mu opioid receptor antagonists : Naloxone and naltrexone

• Mixed agonist/antagonist of the mu receptor and partial kappa agonist : nalbuphine
