Necrolytic Acral Erythema (NAE)

มักพบในผู้ป่วย HCV infection พบได้ทั้งเพศชายและหญิง ช่วงอายุ 20-60 ปี

กลไลการเกิด

ไม่แน่ชัด เชื่อว่า multifactorial

• Hepatic dysfunction
• Hypoalbuminemia
• Hypoglucagonemia
• Hypoaminoacidemia
• Zinc deficiency
• Diabetes with or without an underlying HCV infection

อาการแสดง และ ลักษณะทางพยาธิวิทยา คล้ายกับ Necrolytic Migratory Erythema (NME) บางรายงานตั้งข้อสงสัยว่าอาจเป็น entity เดียวกัน เพียงแต่ NAE มักมีผื่นค่อนข้างจำเพาะคือ เด่นที่ acral area

อาการแสดง

แบ่งเป็น 3 ระยะ

Initial stage :
ลักษณะตุ่มหรือปื้นถลอกสีแดงคล้ำ มีขุย
บรรยายว่า scaly erythematous papules/plaques appear with characteristic dusky or eroded centre (Figure 2)

In the well-developed stage :
ผื่นรวมตัวกันเป็นปื้นใหญ่และหนา ขุยติดแน่น อาจมีคล้ายตุ่มหนอง
บรรยายว่า demarcated thick hyperpigmented plaque with adherent scales, occasional pustules (Figure 3)

Late stage :
ผื่นเห็นขอบเขตชัดขึ้น สีคล้ำขึ้น
บรรยายว่า more circumscribed, thin and hyperpigmented in late stage (Figure 4)

หากดูตามระยะเวลาแบ่งเป็น

Acute lesions : ลักษณะผื่นแดง ตุ่มน้ำขนาดใหญ่ ขอบลอก (erythema, flaccid blisters and erosions at the margins)

Chronic lesions : ลัษณะปื้นหนา ลอกเป็นขุย ขอบแดงหรือน้ำตาลคล้ำ (hyperkeratotic surface, mild erythema and dark red margin)

การดำเนินโรค

Disease duration 2-120 เดือน
Spontaneous remission and relapse

Relationship to HCV

พบว่าผู้ป่วยที่มีผื่นชนิดนี้ >75% ตรวจพบมี HCV infection ร่วมด้วย ดังนั้นแนะนำให้ตรวจ HCV serology ทุกราย
หากไม่แน่ใจภาวะ NME อาจตรวจ glucagon level เพื่อแยกโรค

Dermoscopy

อาจช่วยในการแยกโรคตามตารางแนบ

การรักษา

• Mainstay คือ รักษาการติดเชื้อ HCV ด้วย antiviral therapy
• Oral zinc supplement พบว่าได้ผลดีที่สุด โดยผลการตอบสนองไม่ขึ้นกับ HCV viral load และภาวะ Zn deficiency ทั้งนี้เพราะการให้ zinc สามารถช่วยในแง่ของ anti-inflammatory, immunomodulatory, antiviral and anti-oxidant effects
• Dose 220 mg twice daily นาน 8 weeks พบว่า complete clearance
• Other treatment modalities :
Intralesional and topical corticosteroids
Topical tacrolimus
Phototherapy
