Stewart-Treves syndrome ‼️
อีกภาวะที่เจอไม่บ่อย แต่อาจต้องนึกถึงถ้ามีลักษณะดังนี้

ตัวอย่างเคส หญิง 84 ปี
โรคประจำตัว dyslipidema, major depressive disorder
8 ปีก่อน ได้รับการวินิจฉัย CA brest Rt. S/p radical mastectomy, radiation, and chemotherapy
ผลตัดชิ้นเนื้อเป็น invasive ductal carcinoma

10 เดือน ก่อนมา รพ. มีผื่นลักษณะ indurated poorly circumscribed violaceous plaque บริเวณ upper third ที่ right forearm ร่วมกับมีแขนขวาบวมจาก lymphedema ซึ่งเป็นผลจากการรักษาก่อนนู้น

3 เดือนก่อนมา รพ. เริ่มมีตุ่มและก้อนบนปื้นแดง มีอาการเจ็บและมีแผลร่วมด้วย

ผล skin pathology เป็น complex infiltrative vascular proliferation dissociating the dermal collagen. Atypical vascular spaces, of varied shape and dimensions, were lined by tumefact endothelial cells, with round-to-irregular vesicular nuclei and occasional prominent nucleoli.
Immunostaining for human herpesvirus 8 : negative

จึงได้รับการวินิจฉัยเป็น Cutaneous angiosarcoma

Bottom line:

🔴 Cutaneous Angiosarcoma เป็น vascular tumor ที่พบไม่บ่อย

🔴 อาจพบเกิดได้จาก

1. de novo
2. ผิวที่เคยผ่านการฉายรังสี irradiated skin
3. เกิดในบริเวณผิวที่มีภาวะ chronic lymphedema กรณีนี้อาจเรียกว่า Stewart-Treves syndrome
   เช่น
   ✔️ เคส Breast cancer with mastectomy และเกิด chronic lymphedema ตามมาจากการตัดต่อมน้ำเหลือง
   ✔️ ภาวะอื่น ๆ เช่น lymphatic malformations, chronic infections, chronic venous stasis, morbid obesity, malignant obstruction, and surgical procedures that disrupt lymphatic flow

🔴 ระยะเวลานับจากเกิด lymphedema จนเกิดเนื้องอกขึ้นมา ประมาณ 1-26 ปี

🔴 Pathophysiology : unclear แต่เชื่อว่าเกิดจาก disruption of lymphatic flow ทำให้เกิดการสะสมของ fluids rich in proteins, growth factors และเกิดบกพร่องของระบบอิมมูนในบริเวณนั้น ส่งผลให้เกิด atypical angiogenesis and malignancy ตามมาในที่สุด

🔴 การดำเนินโรค : Aggressive with a high rate of local recurrence & metastases

🔴 การรักษาไม่ค่อยได้ผลดีนัก อาจใช้วิธีต่าง ๆ เช่น Chemotherapy, immunotherapy, and radiation
therapy และอาจร่วมกับการผ่าตัดออก

🔴 การป้องกันการเกิด angiosarcoma นี้คือ ต้องลดความเสี่ยงของการเกิด lymphedema ให้ดี เช่น weight loss, pressure garments, physiotherapy, and compressive devices
